WHAT ARE CHW SERVICES?
Community Health Workers provide culturally appropriate health education, outreach, navigation, and advocacy services to connect community members with healthcare and social services.
Note: Individual CHWs do not/cannot enroll in PAVE.
CBO REQUIREMENTS FOR CHW SERVICES
Must be a public or private non-profite with 501(c)(3) status, OR
A fiscally sponsored entity of a 501(c)(3) organization
Must provide 501(c)(3) documentation
Must have at least one administrative location in California
The CHW must have the 'CHW Certification' or 'Violence Prevention Certification' and you will have to add their information in PAVE for CHW

The physical location associated with your organization's operations, which can include where services are dispatched from or based. This CANNOT be a P.O. box or residential address.

STEPS TO ENROLL FOR CHW SERVICES
1 Obtain your Type 2 NPI (organizational NPI) - for application guidance, see Preparation Steps, Step 4
2 Gather all required documentation - see Enrollment Checklist
3 Create a PAVE account
4 Complete the electronic application selecting "Community-Based Organization" as provider type
5 Upload all supporting documentation
6 Submit application and track status through PAVE portal
